Composition:
Each tablet contains:
Losartan……………………………………50mg.
Hydrochlorothiazide……………………….12.5mg.


Indications:
A combination medication used to treat high blood pressure when Losartan alone is insufficient.


Dosage:
•  Starting dose is 1 tablet (50mg/12.5mg) once daily and can be increased after a minimum of 3 weeks to 2 tablets once daily.


Contraindications:
•  Patients with: high levels of potassium in the blood, renal artery stenosis, low blood pressure, liver problems, mild to moderate kidney impairment, pregnancy or decreased blood volume.


Warnings:
•  Do not take if pregnant or breast feeding.
•  Do not take if patient has: hepatic impairment, renal impairment, gout, diabetes, post-sympathectomy or acute myopia.


Adverse effects:
•  Dizziness or lightheadedness, stomach pain, fatigue, skin rash, runny or stuffy nose, sore throat or dry cough.


Dosage form: Yellow tablet.


Expiry date: 24 months after date of manufacture.


Packaging:
•  Contained in 2 blister packs,12 tablets per blister pack.
•  Contained in 50 blister packs,12 tablets per blister pack.


Storage:
•  Store in original packaging under 25°C, protected from moisture and direct sunlight.
•  Keep out of reach of children.
